Your privacy is very important to me. This policy explains how your personal health information is collected, used, and protected in accordance with the Personal Health Information Protection Act of Ontario and the Standards of Practice of the Ontario College of Social Workers and Social Service Workers.

Collection of Personal Health Information

I collect personal health information directly from you for the purpose of providing psychotherapy. This may include contact information, personal and mental health history, relevant background, session notes, and information needed to support your safety and care. Only information necessary for treatment and clinical documentation is collected.

Use of Information

Your information is used to provide safe, ethical, and effective psychotherapy. This includes assessment, treatment planning, session documentation, coordination of care when appropriate, and communication related to appointments. Records are maintained as required by PHIPA and the OCSWSSW.

Consent

Your consent is obtained before collecting, using, or disclosing your information. You may withdraw consent at any time except where information must be retained for legal or regulatory purposes. Consent is reviewed during your first session.

Confidentiality and Legal Exceptions

Your information is confidential. It will not be released without your permission except in situations required by law, including:

  1. Concern that you may harm yourself
  2. Concern that you may harm someone else
  3. Concern that a child or vulnerable person has been harmed or is at risk
  4. A court order or subpoena
  5. Specific mandatory reporting obligations

These limits of confidentiality are reviewed with you at the start of therapy.

Storage and Security

Your records are stored in secure, encrypted systems that comply with PHIPA. Access is restricted to myself as your therapist. Records are retained for the period required by Ontario law and the OCSWSSW Standards of Practice, and are securely destroyed when no longer required.

Electronic Communication

Email and online messages are used only for administrative purposes such as scheduling. They are not used for therapy. While efforts are made to protect privacy, electronic communication carries inherent risks. You may choose your preferred method of contact.

Online Therapy Privacy

Virtual sessions are conducted through a secure, encrypted, PHIPA compliant video platform. You are responsible for choosing a private location where you feel safe and comfortable. Audio or video recording of sessions is not permitted.

Access to Your Record

You may request access to your clinical record or request corrections to information that you believe is inaccurate. Requests must be made in writing. You will be informed if any part of the record cannot be released due to legal requirements.
